What to Expect at Your Visit

Cardiovascular Consultants welcomes you to our practice. We appreciate you allowing us to participate in your healthcare. To ensure your visit goes as smoothly as possible:

  • Complete new patient forms and return prior to your appointment by fax or mail
  • Bring a copy of your insurance card(s)
  • Bring any referral or authorization required by your insurance
  • Bring a list of your medications including dosage, or your actual pill bottles
  • Arrive 20 minutes prior to your scheduled appointment time

You will receive a confirmation call two days prior to your appointment. If you are unable to keep the appointment, we request a minimum of 24 hours notice to avoid a cancellation fee.
